1 . Scheduled Application Form (Form 2, Rule-5)

2 . Registration copy (code no. 1-3831-0000-1836) and   the original copy of the Treasury invoice for submission of 15% VAT (Code 11133-0000-0311) on that fee.

3 . The applicant signed a minimum of 20 members 03 Width bye-law.

4. The minutes of the organizational meeting held in coordination with 20 members interested in registration

5.  Each of the applicant members has 1 width, newly opened passport size color photograph and mobile phone number.

6.  Affidavit (by the proposed management committee).

7 . Upon the date of application from the beginning of the proposed co-operative society, the cost of collection, expenses, savings and tax filings (if any) are available.

8 . Next 02 (two) year budget estimation.

9 . Co-operative Society Office Rental Contract

10. Other information required by the registrar.
